TELEHEALTH MEMBERSHIP PLAN

For many young and healthy patients who do not need to been seen in the office regularly, we offer a limited number of Telehealth Membership Plans.

HOW DOES IT WORK?

Similar to our other membership models, patients pay a membership annual fee, which includes one in-person annual exam/physical, plus access to our providers via telehealth for other needs throughout the year.

WHAT IS THE COST?

$400 annually includes one in-office exam/physical (required) valued at $90, plus access to our providers via telehealth throughout the year. Regular visit fees are $50 or $90 per in-office visit and $50 per telehealth visit.

WHO IS ELIGIBLE FOR THE TELEHEALTH PLAN?

Patients from 16 – 30 years old who are relatively healthy and generally do not need to see a physician more than once a year, but want the comfort of having access when needed. For example, college-age children who want access to same-day or next-business-day telehealth appointments for illness. Young professionals or young adults who generally only see a clinician once a year for an annual wellness exam, but want access to the practice for times when they develop a sinus infection, need a medication adjustment, or suspect a UTI, for example. Our team will work with patients to determine who is a good candidate, and we reserve the right to not accept patients into the Telehealth Plan if they are not a good fit because of complicated health issues.

WHY DO I NEED AN ANNUAL IN-PERSON EXAM IF I AM A TELEHEALTH PATIENT?

We believe that quality primary care still requires a physical exam. Telehealth is very effective and efficient for managing many health issues, but as your partner in your whole health, we do require one, in-personal annual exam (included in the membership price) in order to be able to provide you high quality, longitudinal care.

*Note that during this COVID-19 crisis, we are able to begin your membership via telehealth and schedule your in-person exam for when restrictions have been lifted and it is safe to come to the office, or at your discretion.
