Our DPC model is designed to provide you with comprehensive, personalized care from providers you know and trust. We offer extended appointment times for in-depth discussions about your health concerns and goals. This patient-centered approach covers a full spectrum of healthcare needs, from urgent care to chronic disease management and preventative services, ensuring all your healthcare needs are met in one place.

What sets our DPC model apart is the expertise of our providers, who have advanced training in midlife women's health. This makes CMOC an ideal choice for women seeking specialized care, as our team is well-equipped to address their unique health needs. Additionally, our team is supported by on-site Internal Medicine physicians, ensuring you have access to collaborative, comprehensive care when needed.

The DPC membership includes an annual sex-specific wellness exam, thoroughly assessing your health status and risk factors. For women, we offer a range of specialized services addressing unique health needs at every life stage.

Understanding the importance of convenience in healthcare, we utilize the Athena Electronic Health Record system, which includes a user-friendly patient portal. This allows you to easily access your health information, communicate with your provider, and manage your care.

the CMoC Advantage

  • CMoC is a fully independent, private medical group that is not owned by a health system, venture capital, or private equity group, allowing us to remain "hospital system agnostic" and refer patients to the most appropriate specialists and providers.

  • At CMoC, our Direct Primary Care is primarily delivered by our team of highly experienced advanced practice providers, consisting of three Nurse Practitioners and one Physician Assistant.

  • Our practice model prioritizes customer service. In-office staff answer calls and coordinate care in ways traditional practices cannot.

  • We encourage our providers to attend at least two professional meetings per year to help them stay current with the latest medical advancements.Our model allows CMoC to remain at the forefront of medical care, quickly adopting new technologies, services, and diagnostics to ensure patients receive the most up-to-date, high-quality, evidence-based care.

  • Our Direct Primary Care Membership operates outside of insurance and Medicare, allowing for complete price transparency.

  • The annual membership fee includes a comprehensive, sex-specific annual wellness exam with a small per-visit fee for additional visits.

  • CMoC has three convenient locations: Mariemont, Mason, and Kenwood.

  • Rest assured, referrals to specialists, emergency room visits, and hospitalizations are billed to your insurance, ensuring you have comprehensive coverage.

  • Labs and imaging services are billed to insurance.

  • Vaccines are available in our offices and are billed to insurance through Vaxcare.

  • We use Athena Electronic Health Record, which provides patients with access to their health information through the Athena patient portal.

THE INVESTMENT

Annual/monthly price based on age ($40, $50, $70 or $90/month) plus a small per visit fee. ($50 for a 30-minute visit, $100 for 60 minute visit)

The initial commitment for our Direct Primary Care program is a 6-month period. After this initial term, members have the flexibility to continue on a month-to-month basis.

We also offer a specialized Telehealth DPC Membership option tailored for individuals aged 16-30. - $500/year

This plan is particularly well-suited for college students and young adults who are generally healthy and may spend time outside the Cincinnati area. This membership provides convenient virtual healthcare access, ensuring continuous care regardless of your location.
