Wearables & Continuous Monitoring

real-time data for precision care

Wearable devices and biosensors allow continuous or episodic monitoring of key physiologic metrics, including:

  • Blood pressure

  • Heart rhythm

  • Glucose trends

How Wearable Data Is Used

At CMoC, wearable data:

  • Provides early warning signals

  • Tracks disease progression or treatment response

  • Enables precision-based management of chronic conditions

This longitudinal data integrates directly into clinical care, supporting timely, personalized interventions.

Monitoring Options

Wearables and continuous monitoring tools provide real-time and longitudinal physiologic data that can be integrated directly into your care. At CMoC, these tools are used to support precision-based management by helping clinicians identify trends, detect early changes, and assess response to treatment over time.

Monitoring may be recommended for short-term assessment or longer-term tracking, depending on your health goals and clinical needs. Your clinician will help determine which monitoring tools are appropriate and how the data will be used as part of your personalized care plan.

BioBeat Blood Pressure Monitor - $150

Biobeat’s cuffless blood pressure monitor is a small chest-worn patch that continuously tracks your blood pressure, heart rate, and other vital signs. The data is automatically shared with your care team, giving your provider a real-time view of your health so you can manage it easily and comfortably from home.

Zio Patch Monitor - $50 place/read - tech billed to insurance

The Zio patch is a small, comfortable device worn on your chest that records your heart’s activity to detect irregular rhythms. You can press the patch and log symptoms if you feel anything unusual. After the wear period, just mail it back, and iRhythm provides your doctor with a detailed report to help guide your care.

Stelo CGM - $100/30 days or $275/90 days

The Stelo Continuous Glucose Monitor is a small sensor that tracks your glucose in real time. You can set it up with a nurse visit or on your own, and the data syncs to the Stelo app to help you make smarter choices for energy, weight, and overall health.
